Why do organisations ask for a statutory declaration for a lost document?
Because it carries criminal weight: knowingly making a false declaration is an offence under section 5 of the Perjury Act 1911, punishable by up to two years’ imprisonment. That makes it strong enough proof of loss for registrars, share registrars, banks and passport authorities to reissue or proceed without the original.
What details should I gather before I start?
The document type and any reference number, roughly when and how it was lost, and what you will do if it is found. If you are replacing a passport, report the loss to HMPO as well — the declaration supports, but does not replace, the authority’s own lost-document process.
