You usually do not need a deed poll to complete an adoption: the adoption certificate is issued in the new name and replaces the birth certificate. A child deed poll is only for a later change that is not already on that certificate, and every parental-responsibility holder must still consent.

Does adoption already change the child’s name?

Yes. When an adoption order is made, a new entry is created in the Adopted Children Register and an adoption certificate is issued in the name the order records. That certificate replaces the birth certificate for passports, schools, GPs and almost every other record-holder. A deed poll is not part of that process.

Do not pay a deed-poll agency to “register” an adoption. The court and the General Register Office already did. If a school or GP asks for proof of the name, show the adoption certificate. Background: deed polls and birth certificates.

When is a child deed poll the right next step?

Only when you later want a name that is not on the adoption certificate: a further surname change after a new relationship, adding or dropping a middle name, or fixing a spelling. That is the same child-deed-poll process as for any under-16, including written consent from everyone who holds parental responsibility.

What about the original birth certificate?

After adoption it is not the document you use for identity. The adoption certificate is. HM Passport Office and schools expect the adoption certificate plus, for a later deed-poll change, the signed child deed and the parental-responsibility consent letters. Keep the paperwork together; do not post your only original.

Adult adoptees who later change their own name use an ordinary adult deed poll from age 16. No adoptive-parent consent is required then.
