This pack prepares documents for one adult using one person's details. If several family members want the same surname, check the evidence each person needs before ordering. A shared surname does not mean everyone needs the same document or a paid pack.

• After marriage or civil partnership: check the marriage or civil partnership certificate route first. HM Passport Office accepts supported double-barrelled surnames too; see the surname guide.
• Adults choosing a new surname: each person using our service completes their own order. Choose only the documents that person needs; our free adult template or the deed poll alone may be sufficient.
• Children under 16: an adult's name change does not update a child's surname. Check parental responsibility and consent before choosing the child route.

These are the limits of our one-person product. HM Passport Office also has guidance for deeds issued on behalf of a family; ask the intended recipient what it needs if you already hold one. A family document does not replace any consent or court authority required for a child's name change.

A declaration may be useful if a receiving organisation specifically asks for one. It is not an automatic requirement for every name change or every Scottish customer. Check the relevant official guidance first. Preparing both documents does not guarantee acceptance; the recipient may require particular evidence, completion or application steps.

If the recipient wants an enrolled deed poll, this pack does not provide enrolment. Read enrolment guidance. For Scotland, start with National Records of Scotland and ask the record-holder what it accepts.

The letter PDF supplies personalised wording for more than 30 types of UK organisation. Add account references, check the recipient's instructions and submit the appropriate evidence yourself. You can adapt the wording for an email where accepted. The pack does not send notifications or complete an organisation's application form for you.
